Dear Miss Zhang,
I'm sorry I can't have the math and chemistry exams today, as I have got a very bad cold. Yesterday afternoon I went swimming and then got a bad headache. My mother took me to the hospital. The doctor asked me to stay at home and have a rest. So I have to ask for two days' leave. I hope I can return to school as early as possible.
